Referrals Print MinnesotaTo refer a patient to the Spinal Cord Injury Rehabilitation Program on the Minnesota campus: Initial referrals: Contact the Referring Referring Provider Office at 800-533-1564 (toll-free) 24 hours a day, seven days a week. Return visits: To request a return appointment for a patient, please call the Department of Physical Medicine and Rehabilitation appointment office at 507-284-5000, Monday to Friday, 8 a.m. to 5 p.m. Central time. Inpatient rehabilitation unit: Please call the Rehabilitation Admission/Outreach Coordinator office at 507-266-6442, Monday through Friday, 8 a.m. to 5 p.m. Central time. Mayo Clinic providers generally can see most patients within two weeks after an appointment request. Patients with urgent issues can usually be seen within 24 hours after their requests. In emergencies, patients are directly transferred to inpatient hospital care.
